Case Approval

 

Case Consideration

To begin a case with the Jordan’s Angel Foundation, it requires a

Caseworker, Doctor from a Hospital, or Social Worker operating directly with the family that needs helps and sees an unmet need. We consider families in need of help to include: those who are in need of financial support and are unable to meet the family’s needs because they are trying to cover family expenses and medical bills.

 

Screening Criteria  

• Children 21 years old or younger

• Family in financial need as validated by the referring organization

• Recommendation of the representative of the referring organization

• Medical authorization of the child's primary physician

 

Required Forms 

• Completed Information/Request Form

• Case Liability Release Form

• Medical Authorization

 

Request Guidelines 

• Gifts, last wishes, and financial assistance to help meet a family’s needs and facilitate the quality of family life

• $2000 maximum per family
